PLEASE DO SO IN A PROMPT MANNER AND GIVE THEM TO YOUR SQUAD MANAGER/CAPTAIN.
Completed forms need to be submitted to Hugh Carty please, no later than
Friday 13 February 2009 (unless they are for players who have joined the Club after this
date)

Data Protection Act
Under the Data Protection Act 1988 it is necessary
for you to give explicit consent for the information given to
London Strollers Hockey Club being stored in a computerised
or manual retrieval system. The purpose of this information
is to maintain membership lists: any medical information will
be given to Club Officials (Team Captains/Managers) as required
for use in emergency situations where the Club is unable to
contact Next of Kin/Guardians. Any disclosure to third parties
will only occur if a medical situation arises requiring a Club
Official to take action. If you have any concerns over the holding
of this information please contact in writing, either of the
Data Controllers, Mr Simon Haspel (Hon. Treasurer) or Mr Terry
Hanson (Club Chair).
By completing the Membership form, you are
giving explicit consent to the information being stored for
the current season.
